Deloitte found that enterprises where senior leadership actively shapes AI governance achieve significantly greater business value than those delegating governance to technical teams alone. Governance isn't a compliance checkbox — it's a value creation discipline.
The Australian Context
For Australian enterprises, these global patterns are amplified by local factors. Our talent market is smaller. Our enterprise organisations tend to be leaner. And the temptation to follow US or UK playbooks without adapting for our context is real.
The OECD's 2025 analysis of AI training across member nations found that while some countries are investing heavily in AI literacy programmes, the vast majority of training focuses on advanced technical skills for AI professionals. What's critically under-served is general AI literacy for the broader workforce — the exact capability that determines whether AI investments generate returns.

A Practical Framework for Escaping Pilot Purgatory
Based on the research and our direct experience with enterprise AI adoption, here's what we recommend for organisations currently stuck in the experimentation phase:
Audit your current initiatives against the six barriers. Be honest about which ones are killing your pilots. If you can't name the specific barrier, you can't address it. Most organisations will find it's barriers 2 (capability) and 3 (workflow) that are doing the damage.
Stop launching new pilots. Counter-intuitive, but launching more experiments when you haven't diagnosed why the existing ones failed is just adding cost without learning. Fix one initiative end-to-end first.
Invest in capability before technology. For every dollar you're spending on AI tools and platforms, ask what you're spending on making your people capable of using those tools. If the ratio is more than 3:1 in favour of technology, you're likely heading toward the 95%.
Redesign the workflow before selecting the model. Map the current process. Identify the friction. Design the ideal workflow. Then determine where AI fits. This order matters.
Measure financial outcomes, not model metrics. The moment your AI KPIs shift from "model accuracy" and "user adoption" to actual business metrics — revenue impact, cost reduction, cycle time improvement — you've made the transition from pilot to production mindset.
The Bottom Line
The 95% failure rate isn't inevitable. It's a symptom of treating AI as a technology deployment rather than a business transformation. The organisations that crack it — the 5% — aren't using better models. They're building better capabilities, redesigning better workflows, and treating AI adoption as the organisational change programme it actually is.
